First page of fax sent repeatedly

Hardware: D/4PCIU
Software: SR 5.1.1 DNA 5 Build 30 FP1
OS: Windows XP Pro SP2

A multi-page TIFF file is sent; only the first page is repeated as many times as there are pages in the TIFF. This has been tried to different analogue fax destinations, so the problem is not on the receiving end.
My software logs the following events:
16:50:00          sendfax  Debug  Start sending
16:50:25          sendfax  Debug  Event:    A4 Dev: 2
16:50:58          sendfax  Debug  Event:    A5 Dev: 2
16:51:32          sendfax  Debug  Event:    A5 Dev: 2
16:52:06          sendfax  Debug  Event:    A5 Dev: 2
16:52:41          sendfax  Debug  Event:    A1 Dev: 2
16:52:41          sendfax  Debug  Done sending

Any ideas?
