Yes indeed, this is a LifeView LR138 (aka FlyVideo 3000 aka FlyTV Prime
33/35), even a genuine one not rebadged to someone else's subystem vendor
name.
Maintainers: Looks like we'll need another PCI ID entry into the saa713x
driver - and yet another OEM name added to this particular card entry.
Leo: Meanwhile, you can force the driver to use the existing card description
by adding "card=2" to the saa713x driver options.
