The Undefined offset: 0 error is already fixed in monotone.

The fact that Apache can eat up the %3F... part is more troublesome, and seems
to be a "feature" in mod_rewrite, e.g.
http://issues.apache.org/bugzilla/show_bug.cgi?id=34602 and earlier reports for
Apache 1.3.x

There's probably some way to avoid this by adapting your rewrite rules
(including the fact that you seem to have 2 internal redirects), but I'm not
enough of an expert on that to give you any hints - Google probably is though :-)
