If I was a gambling man, I'd put my money on the power supply. The tech sheet for this card says up to 1200 ports per chassis, which equates to 10 cards (assuming they base this figure on the 120 port 4E1 card).
Power supply issues have been a problem in the past with many high-power cards. You'd be looking at close to 200W just for these cards alone.
Is there any more clues in the event viewer to say what's going wrong?
